How can I make a New Patient appointment?

For New Patient appointments Please call our office at 313-565-3565 and we will be happy to schedule an appointment.

What should I bring to my appointment?

  • Insurance Card
  • Drivers License or Valid ID
  • Referral (If required by your insurance)
  • List of Medications with Names and Dosages
  • Medical Records
  • Patient Forms

Is there a cancellation fee?

Yes, there may be a same day cancellation and no-show fee.  Please let us know as soon as possible about a change or cancellation of your appointment.

What form of payments are accepted?

Cash, checks and most major credit cards are accepted.

Do I need a referral for an appointment?

Allergy and Immunology is considered a specialty service. Some insurance companies may require a referral to see a specialist such as an HMO.  Please check with your insurance to see if a referral is needed if you are not sure.

What should I expect at my first appointment?

At your first appointment, a history and physical examination will be done. Some appointments are purposefully set up to only accomplish a consultation and testing will be performed on a separate day. Depending on insurance, skin testing, breathing test, or other tests may also be performed.  Review of testing results and medications will also be covered. Please expect to be here for 1-3 hours depending on the amount of testing that will be done.

How much will the visit cost?

Depending on your insurance, we may be able to give you an estimate of the cost. However, this is just an estimate and you need to check with your insurance regarding the cost.

What insurance do you accept?

We participate in several insurance plans and have a self pay option. If you have a question about a specific insurance call us at 313-565-3565. *Please note we are no longer accepting new Medicaid Patients.
